Our goal is to improve the quality of life for community members, allowing them to live independently by:
- Allowing members to remain within their communities while ensuring quality of care.
- Continuously consulting with the community relating to long term care.
- Developing and delivering community support services.
- Providing off reserve Indigenous individuals/families with information and referrals to long term care service providers.

How This Program Works
Content
The Lifelong Care Program provides workers who take a holistic approach to community support services, such as:
- Visiting members at the center, in hospitals, long term care facilities, social and cultural events.
- Providing security checks on the safety and well-being of members.
- Providing information, advocacy and medical escorts.
- Assisting with filling out forms and applications.
- Accessing services from mainstream agencies and organizations.
- Assistance with funerals and wakes, telephone calls on behalf of clients, etc.
- Providing group dinners for community members to socialize, get out of their homes and enjoy a nutritious meal.
- Providing general education through presentations on specific issues of health and wellness.
